Abstract
Nurses play a pivotal role in sustaining primary healthcare systems, particularly in school and community-based programs. Objectives: To examine the impact of the Nurse Incentive Program on nurses’ motivation, data quality and professional accountability. Methods: A structured, performance-based recognition initiative was implemented and nurses’ activities were systematically monitored using a pre-defined evaluation framework. Key performance indicators included accuracy of data entry, engagement in community health activities and adherence to professional standards. Results: Implementation of the program led to measurable improvements in data quality, enhanced professional accountability and increased motivation among nursing staff. Observed outcomes indicated positive effects on both individual performance and overall organizational efficiency. Conclusions: The study demonstrates that systematic recognition programs with clear performance metrics can significantly enhance nurse engagement and institutional effectiveness, supporting long-term growth and improved healthcare delivery.
Introduction
The efficiency and effectiveness of healthcare systems depend heavily on the motivation, accountability, and engagement of front-line workers, particularly nurses, who are the backbone of community and primary healthcare delivery. In many developing health systems, challenges such as low morale, limited recognition, insufficient feedback, and lack of structured incentives often hinder consistent performance and quality improvement, which ultimately impacts patient care and organizational outcomes. In response to these challenges, Hamro Health introduced the Nurse Incentive Program — a structured, performance-based framework designed to acknowledge and reward nursing excellence through measurable, data-driven indicators. The program fosters intrinsic motivation and professional accountability while aligning individual contributions with institutional goals, enhancing service quality, operational efficiency, and overall healthcare outcomes within school and community-based programs. By systematically linking recognition with performance metrics, the program cultivates a culture of continuous improvement and sustained professional development among nursing staff.
Objectives
The primary objectives of the Nurse Incentive Program are to strengthen professional accountability and work discipline among nurses by encouraging them to take ownership of their tasks and maintain consistent, high-quality performance. The program also promotes accurate and timely reporting through the Hamro Health digital system, ensuring data is correctly entered and available for effective monitoring and decision-making. A key goal is to enhance community engagement by fostering stronger connections between schools, parents, and health workers, thereby improving health awareness and participation in programs. Additionally, the initiative recognizes and rewards nurses who consistently perform well or demonstrate innovation in service delivery, motivating continued excellence. Finally, the program supports professional development by creating opportunities for nurses to participate in research, leadership activities, and skill-building initiatives, helping them grow in their careers while contributing to the organization’s overall objectives.
Methodology and Evaluation Framework
The Nurse Incentive Program follows a performance-based evaluation model that integrates both qualitative and quantitative indicators. Each nurse’s performance is assessed periodically across multiple key criteria. These include system activity, reporting, and quality data entry, which measure the frequency and accuracy of completing clinic visit forms and other relevant documentation in the Hamro Health School System, excluding Saturdays. Student data completion is monitored to ensure all required information is accurately recorded and kept up to date, while BMI record completion tracks the submission of BMI forms for all students every six months. The program also evaluates efforts to increase the number of parents using the Hamro Health App, as well as engagement within the HH WhatsApp community, including the sharing and promotion of standard health content. Maintaining positive and consistent communication with parents is another key criterion, alongside disciplinary conduct, which assesses professionalism, punctuality, and adherence to organizational policies. Finally, participation in scheduled virtual meetings, training sessions, and review meetings is monitored to ensure active involvement. Each of these criteria carries a specific weight, providing a transparent and balanced evaluation framework that supports objective performance assessment and continuous improvement.
Implementation and Monitoring
The Nurse Incentive Program is implemented in monthly cycles. Performance data is collected and analysed directly from the Hamro Health digital system to maintain objectivity and accuracy. Regular feedback sessions are conducted to guide nurses on areas requiring improvement and supervisors offer mentorship and support to enhance overall performance. Nurses demonstrating exceptional performance are shortlisted for recognition, with opportunities to be featured on the Hamro Health official platform. This approach aligns with best practices in performance-based incentive programs, which emphasize the importance of continuous monitoring and feedback to enhance healthcare delivery and staff performance.

The Nurse Incentive Program has opened avenues for professional growth and innovation.
